Congestive heart failure (CHF) is a chronic condition in which the heart muscle is unable to pump enough blood to keep up with the body’s need for oxygen. Patients’ vital signs, weight and other symptoms are monitored remotely so that the clinical team can respond quickly to further investigations and prevent further exacerbations that could lead to hospitalization.
